Does this require a SIM card, Or does it have a Verizon compatible eSIM?

This adapter supports Nano-SIM (4FF), Peplink eSIM, and BYO eSIM (2-profiles). US Carrier Certifications are pending for AT&T, T-Mobile, Verizon

Does the device have battery back-up for operation during a power outage?

The POTS Adapter does not have a built in battery backup.
