Do I have renew my license annually?

The OpenVPN license is considered a lifetime license for your device. It does not require annual renewal for the license to remain active for the life of the device.

Can I activate multiple licenses on the same router (Pepwave surf soho mk3) to enable multiple openvpn client profiles?

Unfortunately, no. You can only have one OpenVPN WAN license active on the Surf Soho.
